The immediate loaded immediate implant
“The hole in one”

Many factors have been proposed to interact with the peri-implant tissue, thus influencing long-term stability and esthetic outcome such as quality of peri-implant tissue, implant abutment interface, material and design of abutment, and surgical and prosthetic procedures. A stabile soft tissue situation seems to have a positive influence on the sealing of the peri-implant interface, thus playing a leading role in the maintenance of dental implants.

In esthetically high-risk cases it is beneficial to carry out all required measures in one surgical procedure. Using the one abutment one time technique during the healing phase, the final abutment is placed and left in situ immediately with immediate implant placement. As a result, the wound can adhere to the abutment and a tight hemidesmosomal junction can establish during healing. As a consequence of this technique the peri-implant soft tissue can be preserved as much as possible and also the adjacent bone seems to stay on a higher level to the implant shoulder. Digital planning and advanced processing technologies help to prepare all needed parts for successful immediate implant placement in combination with immediate loading / restoration.

The lecture describes the surgical and prosthodontic steps and shows the advantages but also limitations on a scientific basis.
